Evaluations are Very Thorough
A lengthy information form is emailed to you before your first appointment, and to be completed to the best of your ability.
Initial evaluations are done face to face
and are 2 hours, but may be broken into
2 one hour appointments if desired.
The fee is $600.
Evaluations are generally scheduled
Tuesday, Thursday, & Friday
7:30am - 5pm
Special arrangements possible

Visits are Customized to Your Needs
Appointments can be telemedicine
(virtual visits) or Face to Face Â
10-20min $150,  21-35min $200,
36-50min $250,  51-65min $300
Appointment availability is generally Tuesdays, Thursday, & FridaysÂ
